Navigating the FAA medical certification process can be challenging when your medical history requires additional evaluation, documentation, or ongoing monitoring.
At Waiola Direct Care, Dr. T. Scott McMurry, a HIMS-trained Senior FAA Aviation Medical Examiner (AME), board-certified family physician, military Flight Surgeon, and private pilot, provides individualized guidance and ongoing case management for pilots navigating complex FAA medical certification requirements.
Our HIMS services are available to pilots throughout the United States, with virtual consultations and in-person evaluations in Kailua-Kona, Hawaiʻi.
The FAA's Human Intervention Motivation Study (HIMS) program provides a structured pathway for certain pilots requiring specialized medical evaluation or monitoring.
We assist pilots with certification concerns involving:
Alcohol or Substance Use: Evaluations, recovery documentation, testing requirements, and ongoing monitoring.
Mental Health Conditions: FAA certification concerns involving psychiatric diagnoses or treatment.
Antidepressant Medications: Evaluation and monitoring under applicable FAA protocols.
ADHD History: Assessment of FAA certification requirements, including potential Fast Track pathways.
Special Issuance Authorization: Assistance with FAA documentation, ongoing requirements, and renewal.
HIMS Transfers: Review and potential transfer of established HIMS monitoring cases.
Not every condition requires enrollment in a formal HIMS monitoring program. We help determine which FAA pathway applies to your circumstances.

Your initial consultation is designed to review your aeromedical history, FAA correspondence, and current certification status.
Dr. McMurry will help identify applicable FAA requirements, determine which records or specialist evaluations may be needed, and develop an individualized plan for moving forward.
When appropriate, we coordinate with HIMS psychiatrists, neuropsychologists, treatment providers, and other professionals involved in your certification process.
